THE EFFECTIVENESS OF ENDOSCOPIC HEMOSTASIS IN GASTROINTESTINAL BLEEDING OF ULCERATIVE ETIOLOGY

Abstract

Endoscopy with systematic repeated treatment significantly reduces the risk of recurrence of bleeding caused by peptic ulcer, compared with control patients receiving wait-and-see tactics. The purpose of the study. To study the effectiveness of endoscopic hemostasis in erosive and ulcerative processes of the stomach and duodenum, depending on risk factors. Materials and methods. The results were analyzed in 204 patients in the comparison group and 121 patients in the main group. Conclusion. The proposed method of endoscopic laser retraction of a blood clot on the surface of ulcers of the gastric mucosa and duodenum (with bleeding of type F-Ia-b and IIa-b) provides a good hemostatic effect, reduces the frequency of bleeding recurrence in the group with no drug risk factors from 7.5% (in 8 out of 106 patients in the comparison group) to 1.5% (in 1 of 65 patients in the main group), in the group with symptomatic ulcers from 17.3% (in 17 of 98 patients in the comparison group) to 3.6% (in 2 of 56 patients in the main group; criterion χ2=6,253; df=1; p=0.013).
